Hi everyone, maybe someone can advise?

I had orthognathic surgery on 18 October with an approved pre-authorization. I called Carefirst to make sure, woman told me yes it’s approved and I won’t pay over $XXX amount (my out of pocket max, my plan doesn’t have a deductible). The hospital/clinic got the approval letter of pre-authorization to go ahead with my surgery.

Tonight I checked my claims and it was finally processed, it says I owe $0 but Carefirst is denying all the charges? Is this maybe just a billing error? I’m going to contact the billing department tomorrow as well as Carefirst, but just really freaked me out. Especially considering I can’t afford a $58k bill.

[Added] I called this morning and they said they’re requesting more medical information, I’m assuming to determine necessity (it was, braces wouldn’t have and didn’t fix the deteriorating and debilitating issue).

They’re also processing my provider charge ($12k) who’s in network, but gosh…the whole thing just really sucks and I’m feeling anxious. Everything had been so smooth until now…
