help selecting the best UHC plan out of these?

I’m in Nevada. In the middle of shopping around for a plan for 2023 (currently covered through 12/31/2022 through a marketplace plan).

I anticipate having knee surgery sometime in 2023. I’m looking at UnitedHealthcare short term plans due to its national network which will allow me more options for surgeons outside of NV. The three options that were recommended to me by an agent were:

A (short term medical copay). deductible 2500 | out of pocket max 5000 | coinsurance 80/20 | 6 month premium 1485.18

: doctor visit $50 copay

: urgent care $50 copay no deductible

B (short term medical plus). deductible 2500 | out of pocket max 2000 | coinsurance 80/20 | 6 month premium 1218.24

: doctor visit 20% after deductible

: urgent care $50 copay no deductible

C (short term medical plus elite). deductible 2500 | out of pocket max none | coinsurance 100/0 | 6 month premium 1269.54

:doctor visit 0% after deductible

: urgent care $50 copay no deductible

Obviously there are other differences too but the above were the most relevant ones I could think of. To me it seems like C is the best option for me and A actually seems like the worst. I’m curious why A would be the priciest. Is there something I should look at more closely before picking one? To be clear these are off-marketplace plans subject to underwriting. Thanks.
