How Do Self Employed Get Health Insurance?

I've been trying to find health insurance for a week and it doesn't make sense to me. I think part of it is no one has much advice or comparison because they all go through their employers, I'm self-employed.

I know people don't like when someone asks for too specific of insurance advice, so I just want to know a few general things.

I'm a 26 M. I am engaged to a 24 F. She has no insurance and makes 20k a year. I don't qualify for subsidies. I live in Ohio. What should I reasonably expect to pay for health insurance (premiums and deductible)?

What kinds of things should I look for in health insurance?

Another thing is I'm technically disabled because of a rare neck problem, but it's gone undiagnosed because doctors haven't figured it out yet. They told me not to mention it because it's not understood or diagnosed. Is that a bad idea? I'm gonna need some kind of treatment in the future.
