Is there an Individual/Marketplace policy with useful access to Stanford Healthcare

I have some unusual conditions for which I see specialists at Stanford. Idiopathic osteoporosis is the big one. Up until now, I have been on PPO plans but COBRA from my prior employer is running out and my "new" employer offers only a ludicrous indemnity plan. So it appears that I will have to go to a marketplace of individual plan.

That's expensive but the bigger problem is that Stanford does not accept any Marketplace of individual PPO plans. None at all.

They do accept exactly two Marketplace HMO's, neither of which allows for Stanford primary care:

Kaiser

Valley Med

Will either of these actually work? I've read here and there that Kaiser only gives referrals if their own doctors don't treat the condition at all. Is this true? I know they do treat osteoporosis but they seem to do it quite badly. I haven't heard anything direct about Valley Med's HMO but getting actual healthcare from them is something I want to avoid.
