Trying to see how much I’ll need to pay for a salpingectomy and have three entirely different amounts

Hi! I’d love any insight anyone can get me. I have an independence blue cross plan through my employer and have elected to get a bilateral salpingectomy for sterilization. I found a doctor to do it and scheduled the surgery for October. I am trying to get ahead of any unexpected costs. It’s my understanding that this SHOULD be entirely covered under the Affordable Care Act as a method of birth control, but I have a feeling I’ll still need to pay something.

Note: My plan has no deductible, but does have an out of pocket maximum of $5,000. I have only spent about $340 out of pocket so I still have quite a way to go. First I called the insurance company myself. I was told I’d have to pay $185 co-pay and 10% of the entire cost. That sounded like a lot of money to me. Having had issues with getting correct info before, I then called the man who manages our health insurance plan for our employer. He came back and told me that I would have to pay 10% of the procedure UP TO a total of $185, but he did tell me to call my OBGYN’s office and have them confirm with insurance. The billing department for my OBGYN called back and told me I’d have a $175 hospital copay but that everything was covered 100%.

So I’m totally lost. I don’t know who to believe or what to expect, and I’m trying to plan so I don’t have any costly surprises.

Does anyone know what my next steps should be or who I should talk to to get a FINAL answer? I can provide the CPT code I was given any any other info needed.

TLDR: received three different cost estimates for a procedure that should be 100% covered under the ACA. How do I find out the true cost ahead of time so I can plan for the cost?
