Just some updates, I spent the last 2 weeks trying to communicate with the insurance. Member services sucked and are incompetent, they did an appeal that was done all wrong, took forever for them to respond to, and didn't tackle the issue I raised. I sent another and god knows how long it'll be before that's done.

I need to resume treatment ASAP. The provider's office told me, however, that there is an option to resume which is to go and ahead and sign the "waiver" that says that I would have to pay for it if insurance doesn't pay up. Yet, the provider's office ALREADY says they will end up billing me if insurance doesn't pay. Is it a good idea to sign the waiver? Or is there more to this?
