Hi all, I need help choosing a plan. My employer is offering medical and dental coverage and paying 50% of premium. I’m sorry if this is the wrong subreddit or if I’m missing anything.

Context about me: 24M in Illinois, moving to California for school next year. Haven’t seen a doctor in years. Generally fit, remain active, no health concerns. I just want insurance to do a check-up. My concerns: acne, dental work, and possible lasik in the future. My employer offers me a separate plan for dental, which I will be getting and LASIK I believe isn’t covered in many insurances.

I make $22.50/hr

The types of plans offered:

P506PSN Blue Precision Platinum HMO 107 (https://www.healthinsurancementors.com/wp-content/uploads/2020/10/P506PSN-Blue-Precision-Platinum-HMO-107.pdf): Overall deductible: $0. Rate**: $443.55/month** and I will be paying $221.77/month. My coworkers told me HMO would be the best option for me since I rarely go see a doctor and the only issue is I need a referral for specialists.

S532PPO Blue PPO Silver 120 (https://www.healthinsurancementors.com/wp-content/uploads/2019/10/S532PPO.pdf): Individual deductible: $3250 participating and $6,500 non-participating. Rate: $495.05/month. I will contribute $247.52/month

G534PPO Blue PPO Gold 114 (https://www.healthinsurancementors.com/wp-content/uploads/2020/10/G534PPO-Blue-PPO-Gold-114-for-2021.pdf): Individual deductible: $1,000 for participating and $2000 for non-participating. Rate: $559.99/month. I will be contributing $279.99/month

P503PPO Blue PPO Platinum 119 (https://www.healthinsurancementors.com/wp-content/uploads/2019/10/P503PPO.pdf): Deductible: $250 for participating, $500 non-participating. Rate: $663.42. I will contribute $331.71/month

There is also a dental plan, DILHM44 (https://www.bcbsil.com/plan-docs/grp/dental/blue-care-dental-ppo-dilhm44-2021.pdf). Rate: $39.98/month. I will pay $19.99/month

Links are provided for each above, but they are outdated and I couldn’t find the most recent ones. My enrollment period coverage is 02/01/2022 – 01/31/2023.

To recap, what I want the most is: see a dermatologist for my acne and see a dentist for dental work. I am also moving to go to school, so I don’t know what plan or insurance company to choose because I also want to save up $$.
